The Center for Wound Healing & Hyperbaric Medicine (CFWH) provides management and consulting services to healthcare providers in the area of wound care and hyperbaric medicine. By focusing on the needs of the patients as well as the partners we serve, The Center for Wound Healing & Hyperbaric Medicine is able to bring advanced wound healing strategies to many communities around the country. Under the supervision of the Program Director, the Nurse Case Manager provides leadership and guidance through consultation in Wound Care Nursing according to the standards of the WOCN / NAWCO. Manages and provides care to patients afflicted with disorders of the integumentary system including wounds issues. The Nurse Case Manager is responsible to implement the nursing process within the Center for Wound Healing & Hyperbaric Medicine and the continuum of care. The nursing process is utilized to assess, diagnose, establish a plan with outcome criteria, intervene, evaluate, and document human responses to actual or potential health problems. This practice includes dependent, interdependent and independent functions. Acts within the guidelines of federal and state laws, Joint Commission or DNV ISO 9001 Standards, and professional ethical practices.

Qualifications & Training:

Registered Nurse graduated from an accredited school or college of Nursing. Bachelor’s Degree preferred.
Current state licensure.
Minimum 5 years of clinical nursing experience.
Minimum 2 years of management experience.
Wound Care experience required. Certification in Wound Care preferred.
Certified Hyperbaric Registered Nurse (CHRN) preferred.
Current BLS from approved agency.
Must possess excellent verbal and written communications skills.
